The Cancer Care Diary is part of Cancer Council NSW's Information & Support Pack. The diary allows you to record information about your cancer care in one place. It has chapters where you can:

write down your appointments with health professionals
list your medications
note any symptoms or side effects
record costs associated with your treatment
write down any extra notes, for example, information for a carer.

Information & Support Packs are available free to NSW residents. To request a pack, call the Cancer Council Helpline 13 11 20.
